The Palliative Care Link Nurse is a nurse who acts as a link between specialised Palliative Care Services and the staff and patients of clinical areas.  

Aim is to promote and improve end of life care for patients and their families/carers in acute care that are on a non palliative care ward or patients under CALHN services.  

It also aims to improve care for patients who may not be imminently dying but goals of care are consistent with a palliative approach.  It's focus is on a collaborative approach.
