The aim of this program is to improve the quality of life of patients with Chronic Kidney Disease and End Stage Kidney Disease through Dialysis and Kidney Supportive Care.

It includes 6 modules that provide knowledge for renal clinical staff to implement quality care and support their team members in the delivery of dialysis and supportive care to patients.

Module 1: Introduction to Kidney Supportive Care

Module 2: Recognising deteriorating & frail patients

Module 3: Symptoms burden in patients with Chronic Kidney Disease

Module 4: Dialysis in elderly patients

Module 5: Withdrawal from dialysis treatment

Module 6: Advance Care Planning
